This form is intended to address the McKinney-Vento Act 42 U.S.C. 11435, and must be completed for each student by 10/30 (FINAL DEADLINE). The information you provide is confidential. Your child will not be discriminated against based upon the information provided.

AFCHES Updates 8/31

We know there are many questions surrounding what school will look like this Fall. We do not yet have all the answers, but hope to provide the updates we do have in this email. Please read through to the end and take a look at the attached documents and links, as there are some important updates about the beginning of the school year.

School Calendar:

You will find attached AFCHES’ school calendar for the 20-21 school year. We don’t yet know when we may transition from remote learning to a hybrid model. As soon as we have updated information, we will share it with you. Click Here! 

Health Information: 

Please thoroughly review the attached “AFCHES Medical Letter 20-21 ” for detailed information. This document includes information on requirements for Immunization Records , Health Examination Forms , IEP and 504 Forms (for Diabetes , Asthma , Allergies  or Medications). These forms are due by the first day of in-person schooling and can be submitted in person on this day or via email to crownheightselementary2@achievementfirst.org. Please take time to review this document as there is further important information about what these forms must include.

Attendance Expectations:

It is important that even during remote learning your scholar is attending school virtually every day. Please take a look at the attached document on Attendance Expectations. It provides detailed information on the school’s expectations on attendance during this time. Click Here! 

Lunch Form Information:

Achievement First Crown Heights Elementary School is required by the City of New York to collect a completed lunch form, or official documentation indicating eligibility for free/priced lunch, for every student. To remain compliant with NYC, we must have 100% of our families complete a lunch forms by Friday, September 11th.  The attached document provides further information and a link to fill out the required form. Click Here! 

Campus Portal Information:

Last spring we asked families to set up Campus Portal accounts to view scholars’ progress reports. To this end, we sent every parent an email with information on how to activate and log into this account. We will be sending another email with this information for those who missed it next week. We have also included in this email a family guide to Campus Portal, which provides directions for use and information on Campus Portal’s features. We will be using Campus Portal to post grades and share attendance. You will also be able to use the message center to communicate with us directly and to request changes to household/contact information. We will not be using the assignments function – teachers will explain how students should view and submit assignments after the school year has officially begun. We will also not be using the fee feature, as we do not require any school fees.Campus Portal Family Guide: Click Here! 

Remote Learning Survey:

Families, if you have not taken the remote learning survey provided by Achievement First, which provides the space for families to tell us their preferences in relation to remote and in-person learning, please do so now. If you do not complete this survey, your child will automatically be places in remote learning by default. To take the survey and share your preference, please follow this link.

U.S. Census 2020

It’s CENSUS TIME! The aim of the US census is to count the entire population based on the age, race and sex of each member in a home. This golden opportunity takes place ONCE every TEN years so our time is now! As you may know, Covid and the effects racism have impacted black and brown communities disproportionately. Communities that go “uncounted” remain unheard resulting in the continued demise of lack of resources and capital. This is a step towards demanding resources and equity to reach our neighborhoods. AFCHES Updates 7/17 📑

We hope this message finds you all doing well. We know there are many questions surrounding what school will look like this Fall. We do not yet have all the answers, but hope to provide the updates we do have in this email. Please read through to the end and take a look at the attached documents and links, as there are some important updates about the beginning of the school year. You will find the following information:

Intent to Return Form: 

First, we would like to ask families to fill out the Intent to Return Form found here. We would normally share this with families throughout the Spring as we admit new families to our school. However, because of the great deal of uncertainty for many, we are sharing this now as a means to get any updated information from families. If you know that you will not be returning with us next year, please let us know on this survey and fill out our online withdrawal form. We do know that many are not fully decided on what their plans will be, and there is a space to reflect this in the survey and provide us information on your current situation. Retrieving Scholar Items from School: 

If you believe any of your scholar’s items from the 19-20 school year are still in the building, please fill out this form so we may attempt to locate these items on your behalf once the school building is open again.

Health Information: 

In terms of updated health information for the 20-21 school year, please thoroughly review the attached “AFCHES Medical Letter 20-21” for detailed information. While we will have further, and much more detailed, information on health precautions in the weeks to come, we wanted to share with you what we do know at this point:

  • There will be thermometers for temperature checks of all people entering the building.
  • Everyone will be required to wear a mask in the building.
  • There will be hand sanitizer stations in every room and in all common spaces.
  • Face shields will be used by more visible staff members.
  • Social distancing will be in place, with no more than 15 bodies in a classroom.

Transportation:

We are still waiting on DOE and state guidance on what bussing will look like next year. Once we have that information we will be sure to share with you ASAP.

Principal Communication:

Finally, you will be hearing from our new principal, Dr. Sade Johnson, by Friday July 31st. She will be sending out a survey to ask parents about their preference for remote versus in person learning for their scholar.

We know that there are many more questions to answer and we will provide as much information as possible as soon as we can. To this end, Dr. Johnson will also be holding a virtual town hall the week of July 27th to discuss next year and what it will look like with families. The exact date and time of this event will be shared in the coming weeks.

First Day of School

School Uniform

**Please hold off on purchasing uniforms at this time. We are still awaiting network guidance on what the uniform policy will be for the 20-21 school year.

Health Forms

It is required that an updated Health Form be provided to the school every year. Please use the summer months to make doctor appointments, so that we have accurate health information for your scholar upon the start of the school year. Updated Health Forms and Immunization Records are due 9/2/20.

All scholars ages 2 month- 18 years in NYC must get the following vaccinations in order to attend. The following are the age five vaccines: Dtap, IPV, MMR, Hep B, Varicella for chickenpox, PCV, Hib. As you know, there was recently a large measles outbreak in NYC. Scholars who do not have the required vaccines by the deadline will be excluded from school. Meaning if they are eligible for the vaccine and do not receive it, they wont be allowed to attend.

If your child has a health condition, they may benefit from taking prescribed medicine at school or a skilled nursing treatment. Talk to your child’s health care provider. Ask if taking medicine at school or a skilled nursing treatment would help your child better manage his health condition…

All students with a diagnosis such as asthma, allergies or diabetes should submit a Medication Administration Form to their school.

Submitting a Medication Administration Form enables the school nurse to treat your child in the case of a medical emergency. Even if your child can take his/her own medicine, it is important for the school clinical team to be aware of the student’s medical needs in case of an emergency.

They are accepted on a rolling basis, but you should submit as early as possible so that we can best meet your scholar’s needs. You must also submit a new Medication Administration Form each school year. The present school year should be at the top of the form.
